Any incident or accident during the transfusion of a blood product must be reported to the Ministry of Health with the following basic information: “please download the Notification Form (FR) from the Health Portal (SANTE.lu)“.
The file is sent by the Ministry of Health to the Blood Transfusion Centre for investigation of the blood products involved. A report is drawn up by the Blood Transfusion Centre and sent to the Ministry of Health; a copy of this report is also sent to the doctor in charge of the transfusion, who reported the transfusion reaction or adverse effect.
